This policy explains what information the Eclipse Mental Wellness, PLLC website collects, what it does not collect, and how we handle a message you send us through it. It covers this website only. How we protect your health information once you are a patient is described in our Notice of Privacy Practices.
Please do not send health information through this website. The contact form is for scheduling and general questions. If you are a current patient, use the patient portal or the Spruce app for anything about your care. In an emergency, call 911, or call or text 988.
Information we collect
Information you give us
The only information this website collects from you is what you type into the contact form: your name, your email address, and your message. The form is delivered to the practice by email. This website has no database of its own and is not connected to our medical record system.
Information collected automatically
Like most websites, our hosting provider keeps standard server logs, such as the IP address and browser type of each visitor and the pages requested, for security and to keep the site running. We do not use these logs to identify you. We do not use analytics tools, tracking pixels, or advertising trackers on this site.
Cookies
This website does not set cookies of its own and does not use advertising or analytics cookies. Sites you reach by following a link from here, such as the patient portal or Headway, have their own cookie and privacy practices.
How we use information
- To reply to you, schedule an appointment, or answer your question.
- To keep the website secure and working.
- To meet a legal obligation, if one applies.
We do not sell information from this website, share it with advertisers, or use it for marketing.
Who has access
Messages sent through the contact form are read and answered by the practice. Our website host, Netlify, delivers those messages to us and holds a copy of each submission until we delete it. Netlify processes that information only to provide its hosting and form service. We may also disclose information from this website if the law requires it.
Links to other sites
This site links to services we use with patients, including Headway for booking and insurance billing, the Charm Health patient portal, the Spruce messaging app, Psychology Today, and our social media pages. Those sites have their own privacy policies, and this policy does not cover them.
How long we keep it
A message sent through the contact form is kept only until your request has been handled. Once it is resolved, it is deleted from the practice’s email and from the website host. If you become a patient, information that is relevant to your care is recorded in your medical record, which is governed by our Notice of Privacy Practices and by medical-records law rather than by this policy.
Children
This website is not directed to children under 13, and we do not knowingly collect information from them online. The practice sees patients aged 14 and older, and care for anyone under 18 is arranged directly with the practice by a parent or guardian.
Your choices
You can ask us what information we hold about you from this website, ask us to correct it, or ask us to delete it. This applies to website and email correspondence only. Medical records cannot be deleted on request. We are required to keep them for the period set by law, and your rights over your medical record, including the right to see it and get a copy, are described in the Notice of Privacy Practices.
Changes to this policy
If we change how this website handles information, we will update this page and change the effective date at the top.
